Welcome to the MentorPath® Registry Profile Intake. Please fill out the fields below so that we can accurately profile you and your coaching expertise.
All required fields are indicated.
You may submit up to 3 photographs, but only 1 will be used. The ideal size of the photograph is 600 x 600 pixels with a resolution of 300 dpi. Don’t worry if the resolution is lower, however, be sure that your image is sharp. Ideally, your image is scaled to match the sample shown. But if it isn’t, then we will resize and crop accordingly.
Your email address is noted as required, but if you do not wish to have it posted visibly on the registry, then indicate that in the SPECIAL INSTRUCTIONS section of the intake form.
PLEASE NOTE: There is a possibility that we will edit subjective information to maintain registry consistency. However we will alert you to changes to be sure they are agreeable.

PORTRAIT TIPS
- Wear clothes that reflect your personality.
- Be sure your clothes are pressed, clean, and free of lint.
- Dress to complement the environment where you will be photographed. For example, white shirts disappear into white walls and active backgrounds compete with busy, patterned shirts.
- Wear colors that bring out your eyes. For example, if you have sparkling baby blues, then wear blue to draw attention to them.
- Don’t overdo foundation, blush, or eye shadow.
- Manicure facial hair, unless a bushy beard is a characteristic style.
- Use powdered foundation to absorb facial oils and reduce shine.
- Show a hairstyle that you usually wear.
- If you wear glasses, then be sure your eyes are not hidden by reflections of the camera lens or environment.
- If you’re a big smiler, then smile wide. If you’re not, then don’t force one. Look natural. A gentle grin is inviting, too.
- Avoid looking stern, severe, scattered or bored.
- Choose your background carefully—it should not be too busy. You are the star of the show!
